We are looking forward to seeing you at your upcoming orthodontic consultation. In preparation for this appointment, please familiarize yourself with our appointment policies below:

New Patients

• New Patient forms may be completed on the day of or at the following link: New Patient Forms.

• Insurance information must be obtained 2 days before the scheduled appointment date to avoid cancellation.

Appointment Prep

• Appointment confirmations are required to avoid cancellation. You will receive a text and/or voice confirmation 1 week before the scheduled appointment date.

• Orthodontic Consultations are permitted to be cancelled/rescheduled up to 1 time. We do request 48 hour advance notice for all cancellation/reschedule requests, if possible.

• No call/no shows are ineligible to reschedule Orthodontic Consultations.

Day Of

• Please arrive 15 minutes prior to appointment time.

• Patients’ legal parent/guardian must accompany patient to appointment.

• Patient/parent/guardian must have physical copy of photo identification.

• No additional parties, besides patients, are permitted in the treatment area, with the exception of orthodontic consultations.
