Please familiarize yourself with our office policies below:

Appointment Prep

• Appointment confirmations are required to avoid cancellation. You will receive a text and/or voice confirmation 1 week before the scheduled appointment date.

• Parents/guardians may cancel/reschedule appointments up to 2 times. We do request 48 hour advance notice for all cancellation/reschedule requests, if possible.

• Insurance information must be obtained 2 days before the scheduled appointment date to avoid cancellation.

• In-date x-rays must be obtained before scheduled appointment date to avoid charges on date of service.

Day Of

• Please arrive 15 minutes prior to appointment time.

• Patients’ legal parent/guardian must accompany patient to appointment.

• Parent/guardian must have physical copy of photo identification.

• Only 1 parent/guardian may accompany patient to treatment area (no additional parties are permitted).
